| The Host Organization, in administrating this National "C" course, will: |

Not conduct the course without U.S. Soccer Federation approval of dates, site and Staff Coaches. |
| |
|
|
| |
|
In the event of any changes to staff, location, or dates, submit an amended Host Application for approval before proceeding with the course. |
| |
|
|
| |
|
Obtain comprehensive general liability insurance in the amount of $1,000,000.00 (one million dollars), naming the United States Soccer Federation as an additional insured. We must have a copy of this in our office prior to the start of the course. |

|
| |
|
Employ at least one U. S. Soccer Federation National Staff Coach for every 14 candidates, maintain that ratio in all course sessions, have a minimum of 28 and a maximum of 56 candidates for the course and ensure that instruction is in accordance with U. S. Soccer Federation curriculum as found in the National Staff Coach Instructor’s Manual. A "C" course may be conducted with less than 28, but never fewer than 22 candidates. However, the State Association will be responsible for the full payment of 28 candidates. |

Take financial, administrative, and legal liability for conducting the course. |

Utilize the attached U. S. Soccer Federation National Coaching School Application form and ensure all candidates meet the eligibility requirements. Applicants must be at least 18 years of age and must have held a National "D" license for a minimum of twelve (12) months. A copy of the National "D" license must be attached to the application. All waivers must be approved by US Soccer to be accepted. |

Utilize standardized U. S. Soccer Federation examination forms, cumulative reports, laws books and candidate workbooks. All paperwork will be sent to you once we have a guaranteed number from you. You must call Kati Hope (312/528-1231) three weeks prior to the start of the course with your guarantee number. You will be charged at $150.00 per candidate, with a minimum of 28 candidates. All materials are sent via UPS ground, please be advised that it may take up to one and a half weeks to receive materials. If materials are not used you must return them to US Soccer or you will be billed $25 per set of books

Remit $150.00 per guaranteed candidate for administrative fees to U. S. Soccer Federation National Coaching Office along with all course applications, examinations, and scoring paperwork immediately after the conclusion of the course. Results are not processed until US Soccer receives full payment. |

Inform candidates that the National Office will process all candidate course work, and mail the appropriate correspondence, certificates, badges and passes to the coaches. This process takes 4-6 weeks after we receive final information. |

Pay each national instructor the course salary of $1,500.00, and pay the "Lead Instructor" a salary of $1,600.00. You will also be responsible for all costs associated with travel, accommodations, and meals for each staff coach. If the Staff member is full time with US Soccer the payment must be made to US Soccer and mailed to the attention of Roland Hahn. |

Applicants must be at least 18 years of age and hold a National "D" License for a minimum of 12 months. A copy of the National "D" License must be attached to each candidate’s application. The National "D" License certification may be waived under one of the following conditions:

Applicant received a NSCAA National Diploma and has held it for a minimum of twelve 12 months. (copy of diploma must be attached to application). |

|
Applicant has three (3) years coaching experience, AND, either three (3) years playing experience with the U.S. National Team or three (3) years professional playing experience. |

| WAIVERS WILL ONLY BE GRANTED BY THE OFFICE OF THE COACHING EDUCATION PROGRAM AT US SOCCER. |
| |
| STATE DIRECTORS OF COACHING, NATIONAL STAFF, OR INSTRUCTIONAL STAFF COACHES MAY NOT GRANT WAIVERS. |
