  State Hosted "C" License Host Checklist

  1. Sight Identification and Preparation

    1. One (1) lined field per 32 candidates

      1. Equipment needs:
      2. Portable goals
      3. Extra balls
      4. Scrimmage vests of 3 different colors
      5. Field markers, cones, flags
      6. Extra water, ice, medical kit
      7. Restroom facilities

    2. Fieldhouse back-up in case of inclement weather

    3. Classroom needs:

      1. Lecture facility
        - Desks/tables & chairs
        - Overhead projector
        - Blackboard
        - Video equipment (TV & VCR)

      2. Smaller classrooms for evening breakout rooms (one room for each staff coach) and one room for each staff coach for oral exams.

      3. Easy, flexible, accessible

    4. Extra personnel:

      1. Athletic trainer (mandatory)
      2. Custodian to open buildings?
      3. Ask instructor if extra assessors will be needed for field evaluations

    5. Secure players

      1. Please review the coordinator schedule for exact needs.

    6. Secure dorm or hotel for candidates

      1. Have candidates to make hotel arrangements themselves
      2. Inquire about group rates from a local hotel

  2. Communications with US Soccer

    1. Contact Kati Hope at USSF, Coaching Department, 312-528-1231.

      1. Receive Staff Assignments from U.S. Soccer
        1. Contact Lead Instructor
      2. Verify guaranteed number of candidates three weeks from start date.
      3. Send copies of Application form and all information sent to candidates to Coaching Department Attn: Kati Hope
      4. Receive paperwork from USSF; Course manuals, exams, etc

  3. Communications with Candidates (Before Applying)

    1. USSF National Coaching School Application
      (Example included with packet and is available via email)
    2. Cover letter informing candidates about course information
      1. Dates, location, cost, application procedure, (see attached sample)

  4. Communications after Application is received

    1. Confirmation letter, (see attached sample)
    2. Send final information packet after balance of payment received

      1. Candidates workbook
      2. Maps, housing information
      3. Additional paperwork from USSF as needed
      4. Schedule and informational cover letter

    3. Send all information to instructors (maps, schedules, housing, etc.)

  5. Communication with Instructors

    1. Make their boarding and travel arrangements
    2. Send them maps, schedules, etc., as needed
    3. Call frequency program to ensure all needs are met
    4. Secure forms, exams, paper, handouts that need to be copied.
    5. Arrange for expenses

      1. Will receipts be collected?
      2. Will a per-diem be given for meals, etc.?
    6. Keep paperwork from candidates organized alphabetically to give to head instructor at the beginning of the course, (with 1 passport size photo of each candidate)

  6. Registration at the beginning of the course

    1. Candidate Sign-in
    2. Supply instructors with a list of participant names, phone numbers and addresses
    3. Additional items for consideration

      1. Coffee & snacks for opening meeting/lecture
      2. Extra maps of the facility
      3. Parking regulations
      4. Emergency phone numbers for family members to call if needed
      5. Be available to answer question

  7. After the Course

    1. Thank you letters to instructors, extra personnel, USSF, host sight
    2. Arrange for payment for instructors and additional support staff
    3. Remit payment to the Federation for $150.00 per candidate

  8. Guidelines for Expenses

    1. National Staff Instructor payments

      1. $1,600 for the Lead Instructor and $1,500 for all others
      2. Please note that if an instructor is full time employee of US Soccer, then the payment is made to the Federation, attention Roland Hahn.
      3. All other staff are paid directly by State Association.

    2. National Staff expenses, (travel, lodging, meals)
    3. Rental for facilities
    4. US Soccer administrative fee per guaranteed candidate of $150.00
    5. Postage - mailing/advertising
    6. Salary for athletic trainer
    7. Misc., expenses, office supplies
