New 2004 Registration Form Available

CHICAGO (Wednesday, October 15, 2003) - A fillable PDF version of the 2004 Referee Registration form is now available on the U.S. Soccer web site. This PDF version will allow a person to type in their information, print a copy and turn it in to the appropriate person. This may be helpful to those individuals who prefer the form typed instead of hand written.
