- Faxed or emailed applications will not be accepted.
- One (1) passport-size photo stapled to the application.
- $100.00 deposit is required to begin the application process.
- Credit Cards (Amex/Mastercard/Visa) are accepted. The entire tuition will be charged if a credit card is submitted.
- Money Order, Certified Check or Company/Club check are also accepted. Please print your name and license level you are applying for on the check.
- National “B” License applicants that do not currently hold a U.S. Soccer National “C” License must submit a copy of their NSCAA Advance National Diploma.
- Waiver applicants must submit proof of eligibility. See application for more details.
- You may still submit an application if you have misplaced your CoachesNet Member ID or U.S. Soccer License number.
