  • Faxed or emailed applications will not be accepted.
  • One (1) passport-size photo stapled to the application.
  • $100.00 deposit is required to begin the application process.
  • Credit Cards (Amex/Mastercard/Visa) are accepted. The entire tuition will be charged if a credit card is submitted.
  • Money Order, Certified Check or Company/Club check are also accepted. Please print your name and license level you are applying for on the check.
  • National “B” License applicants that do not currently hold a U.S. Soccer National “C” License must submit a copy of their NSCAA Advance National Diploma.
  • Waiver applicants must submit proof of eligibility. See application for more details.
  • You may still submit an application if you have misplaced your CoachesNet Member ID or U.S. Soccer License number.

Notification of application status will be sent via e-mail.
