The Let’s Play! program from the Maryland State Youth Soccer Association (MSYSA) expands soccer opportunities for children in Title I and community elementary schools. A core component of the program’s success is its commitment to performance assessment and data collection.
This resource pack provides templates and tools for collecting feedback and evaluating program quality. Resources include registration data forms, parent and player end-of-season surveys, coach-mentor feedback surveys, and site evaluation forms. Maryland State Youth Soccer Association (MSYSA) is the state’s representative of U.S. Soccer and U.S. Youth Soccer, serving over 150,000 players and 350 member organizations.
