Since 2006, Guzan has established himself as the second-choice goalkeeper for the United States national team behind Tim Howard. He made his World Cup qualifying debut in the second leg of a second-round qualifier against Barbados.

When Howard was suspended for a match at El Salvador, Guzan started and helped earn a critical 2–2 draw. He also started in the 3–0 win over Egypt in the 2009 Confederations Cup. In 2010, Guzan was named to the U.S. national team's provisional 30-man World Cup roster

Since then, Guzan patiently remained a back-up until Howard was recovering from a back injury in 2013. He stepped-in and started in two World Cup qualifiers, he didn’t allow a goal during either match and picked-up a 1–0 win over Costa Rica and a historic 0–0 draw against Mexico.

Guzan was also capped for the U.S. during the 2015 CONCACAF Gold Cup, as Howard decided to take a break from international soccer following the 2014 World Cup. Both Guzan and Howard were capped for the Copa América Centenario, but Guzan was chosen as the starting keeper.

In 2017, Guzan signed a contract with recently formed MLS team Atlanta United on a free transfer and then returned to the US following the conclusion of the 2016–17 Premier League season. After nine years in English football, he left Middlesbrough on June 10, 2017.

During the 2017 MLS season, Guzan started 14 games, he led the league in save percentage (79.2%) and won the MLS Save of the Week three times. In 2018, Guzan won the MLS cup with Atlanta United after a 2-0 victory over the Portland Timbers.
